New Pediatric Patients
Pediatric Associates of Brockton welcomes new patients. Choosing the right pediatrician for your child is one of the most important decisions you will make. By selecting Pediatric Associates of Brockton, you can feel confident and comfortable that you have made the right decision. Please complete the Newborn Getting To Know You Form Request or the New Patient transfer request on our site and someone from our team will reach out within 48-72 hours.

What to bring to your first visit
We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite the first appointment, we ask that you please have your child's medical records transferred to our practice prior to his/her appointment. Also, please be sure to bring your child's medication information (bottles preferred), insurance card,
co-payment and all pertinent information that will help us provide appropriate care to your child.
New Patient Forms
We are also happy to offer you the option to download, print and complete our office forms from home to reduce wait time during your first appointment. Please complete the forms and bring them with you.

What to Expect​
On the day of your child's first visit to our office, we recommend arriving 15 minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. We look forward to meeting you and joining with you to help your child maintain optimal health.
Insurance and Billing
We accept most HMO, PPO and POS insurance plan are accepted in addition to MassHealth, Tufts ACO and most private insurances. Please contact our billing office with any questions (774) 776-1163.
